2009 Conference and Membership Registration Form

(Return no later than January 4, 2009 for Pre-Conference Rates)

Pre-Registration is a 2 step process

     1. Submit the Membership/Registration form in one of three ways (see below)

2. Submit your payment (by check or by credit card [PayPal])

Membership/Registration Form:  Select one of the following methods to pre-register for the conference.  CONFERENCE PRE-REGISTRATION FORMS MUST BE RECEIVED BY JANUARY 4, 2009.  For registration after January 4, 2009, you may register at the door for $110.00. The Wednesday evening Social, yearly membership dues, and access to the online proceedings are included in the registration fee.

1.  By e-mail:  Complete and submit the form felow.  The submit button will send an e-mail message to the WPGRS business manager with the form contents.  If you elect to pay by personal check, mail your payment to WPGRS, P O Box 178029, San Diego, CA 92117.

2.  By US mail:  print out a pre-registration form, complete it, and mail it with your payment to WPGRS to WPGRS, P O Box 178029, San Diego, CA 92117.

3.  By FAX:  print out a pre-registration form, complete it, and fax it to 858-581-2859.  You may submit your payment mail (address above), or by using the PayPal service at the bottom of this page.
